Community Connectors is a Social Prescribing service which you can self refer to, working in partnership with GP Practices across Hastings, supporting you to improve your health and well-being. We help you to develop personalised and practical solutions to everyday issues. Social Prescribing helps you to explore services, groups and activities that can support you to improve your health and emotional well-being. Addressing difficult issues needs extra time and support.

Healthier You – Diabetes Prevention Programme

To be eligible for this programme your HbA1c must be between 42-47 mmol/mol or Fasting Plasma Glucose between 5.5-6.9 mmols/l and dated within the last 12 months. Alternatively, if you have a history of Gestational Diabetes you are also eligible, as long as you have a blood test dated within the last 12 months, which is not indicative of Type 2 Diabetes.

If your GP has sent you a letter or text which confirms that you are at risk of Type 2 diabetes, you are able to register for the Healthier You: NHS Diabetes Prevention Programme. To be able to register online you will need the following information found in your letter: Your NHS number, latest HbA1c or FPG reading, including the date and your GP surgery name

You can self refer to The Community Bladder and Bowel Advisory Service who provide a comprehensive and proactive service to meet the needs of men and women who have complex continence problems. The team implements education and training programmes, on all aspects of continence care for health professionals, social services and the independent sector.

What do they provide?

  • Advice and guidance
  • A contractor to supply products for adults and resident in care homes
  • A home delivery service
