All you will be asked to do is reply to some short questions occasionally and meet with us up to 10 times a year.
We would like to gather as many patients as possible for this group so that we get a good representation of patients. So whether you are young or old, working or not working, healthy or have long term conditions, we want to hear from you all.

By completing this form you are giving consent for a member of the PPG to contact you and your personal details such as Name and Email address will be shared with the Chairman of the PPG , please note PPG members are not members of the NHS and are not employed as surgery staff.
All prospective members will need to read the The Hill Surgery Patient Participation Group Confidentiality Policy, Conflict of Interest and Declaration Agreement for Volunteers and you will be required to sign the consent form with the Chairperson before you come to your first PPG group meeting.
